Parental Consent: One parent or legal guardian must consent to the procedure. The minor patient must consent to the procedure. Minor patients will need the 24 Hour Consent Form.
ID requirements:
- Parents or guardians must have a government-issued unexpired photo ID (choose one)
- Driver’s license
- State ID
- Passport
- Parent must have (choose one)
- Original birth certificate (parent signing consent must be listed on birth certificate)
- Tax forms with minor patient listed as their dependent
- Guardian must have
- Original court-issued papers granting guardianship of the minor patient
- Minor must have an unexpired photo ID (driver’s license, state ID, or passport).

Judicial Bypass: Minor patients who choose not to involve their parents or guardians must obtain a judicial bypass per State of Michigan regulations. Minor patients will need the 24 Hour Consent Form as well.
To obtain a judicial bypass, minor patients should follow these steps:
- Call our appointment coordinators at (313) 538-2020 and go through an appointment intake. Our counselors will describe the following steps and answer any questions.
- Contact the lawyer that we provide. Because of COVID, this will be a remote phone call.
- Appear in front of a judge to determine if the minor can consent to the abortion. The judge will provide a digital raised seal document that must be brought to clinic on appointment day. Because of COVID, this will most likely be a remote video hearing.
There is no fee for the attorney meeting or the bypass. The judicial bypass process takes about 1 week on average.
